DOMESTIC* Application Procedures

Please complete the following checklist to ensure that all requirements associated with your application are completed.

I have enclosed the following documents:

Checklist– One original and one photocopy of your completed application must be submitted.

   A completed application for research candidature/scholarship form.

2. 2.

   Full Curriculum Vitae (including details of publications).

3. 3.

   Original or certified copies of academic transcripts (for institutions other than James Cook University) – including evidence of graduation for each completed degree/diploma.

4. 4.

   A copy of the tertiary institution's official grading system (if it is not specified on the transcript).

5. 5.

   A two-page outline of proposed research topic with clearly stated aims.

6. 6.

   Evidence of Australian/New Zealand citizenship or Australian Permanent Resident status.

I have done the following:

   Contacted two academic referees and given them report forms to complete.

2. 2.

   Made arrangements for an offer of award to be acted on immediately if I am not at my mailing address between 15 December 2008 and 14 February 2009.
