Forms to Apply for Access or Changes to Administrative Systems

Requests for access to or for changes to be made to the Administrative Systems: can be requested by downloading, completing and forwarding the appropriate forms.


Forms for Computer Accounts

Change your password
This form allows you to change your central Unix password which allows access to your email, off-site web browsing, and file sharing of Unix directories on personal computers, and allows login to your Unix account.
External Account - User Registration Form
This form should be completed to obtain a computer account on the central unix system. All visitors to JCU will be required to complete this form if they wish to access computing facilities. A charge code MUST be supplied with the application as a fee is applicable.
Delegate Account Registration Form
This form should be used to register for a delegate account. Delegate accounts are similar to external accounts (see above), with the additional benefit of having a nominated account manager who controls access to the account via an online administration system.

Other Electronic Forms

Apply for 'Restricted Site' access
JCU staff and students Internet access to restricted sites is normally blocked. It is recognised that some users have valid academic or administrative reasons for visiting a restricted site. Such users may use this form to request their Head of School authorise their access to specific restricted sites.
Apply for 'Official Server' status
It is a requirement of the Queensland Auditor's office that only 'official' JCU Webservers be visible to the outside world.

This form also covers making any other service (eg: SSH, FTP) available from off-campus.

Miscellaneous Supplies Requisition Form
JCU staff may requisition miscellaneous supplies and services using this form. Available supplies include software, laser printing and manuals.
Printer Queue Requisition Form
Information Technology representatives for Faculties & Divisions can request printer queues to be set up on the central systems.
Request a new IP address.
Request a host to be added to DNS. here.
Decommission a host and remove from DNS. here.

Telephone Installation or Alteration [Word.doc]

Telephone Account Code (TAC) [Word.doc]

ITR Central Computing Bulletins Gateway
This eform is used by System Administrators to formulate a bulletin that is to be posted to the Central Computing Bulletin board.
