The human ethics application forms have been updated and must be used when submitting for human ethics approval

THIS INFORMATION IS FOR THOSE STAFF AND STUDENTS WHO USE HUMAN ETHICS APPLICATION FORMS INCLUDING THE ABORIGINAL AND TORRES STRAIT ISLANDER ETHICS FORM.

The human ethics application forms have been updated and must be used when submitting for human ethics approval. If you have already started an application on the old form we will have a grace period up to March 8th 2017. If you submit on an old version of the human ethics application form after this date we will not accept the outdated form and will ask that you resubmit on the updated form. The updated forms are marked in the footer Jan 2017 so you will be able to tell if you are using the correct form.

UPDATE ON HOW TO SUBMIT:

There is no longer 2 separate forms for human ethics. Previously there was an application form for Low/Negligible risk and an application form for full HREC review. There is now one form for both ALTHOUGH if your study is Low/Negligible Risk you will need to submit your application with a completed Low/negligible Risk coversheet. The information sheet, consent form, amendment form and report forms have also been updated. The Aboriginal and Torres Strait Islander form has also been updated and the updated form must be used also.
