You must read and accept all conditions in the following declaration and authority before submitting your application. Please note upon submitting your application you are also agreeing to the Terms and Conditions outlined below.

Your application cannot be considered unless you accept the conditions as indication in the Scholarship Acceptance form and indicated below:

• declare that, to the best of your knowledge, all information supplied in the application is true and correct.
• recognise that it is your responsibility to provide all the necessary documentation to support this application.
• authorise JCU to verify any information provided by you.
• are aware that your application will be considered for the JCU Access Fund only.
• are aware the information you have provided may be shared with staff involved in the selection process.
• understand that giving false or misleading information is a serious offence under the Criminal Code Act 1995 (Commonwealth).
• Are aware that if you have a disability, injury, illness or health condition, you will be contacted by the Student Equity and Wellbeing Unit to talk about the support services available to you during your studies.

Research Scholarship Applicants:

QUALIFICATION SEARCH & INFORMATION VERIFICATION

I authorise James Cook University to obtain further information with respect to my application and, if necessary, seek academic information or transcripts from Australian or overseas educational institutions. Where necessary QualSearch will be engaged to access this academic information. I understand that James Cook University is not responsible if any educational body / institution does not supply these records. I understand that the results of the search will be made available to me on request and that an audit of this authority may also be undertaken.

I authorise James Cook University to contact the referees named in my application to seek additional information or clarification. I acknowledge that James Cook University may seek to verify any publications listed in which I am named as an author by contacting the publisher, editor or co-authors and may seek to contact staff from the Educational Institution where any research projects, theses or dissertations referred to in this application were obtained.

I declare that the information I have supplied in this electronic form is, to the best of my knowledge, complete and correct. I acknowledge that my application is subject to acceptance by James Cook University and that any offer for a scholarship may be subject to additional conditions.

INFORMED CONSENT for DOMESTIC APPLICANTS

I understand that:

• James Cook University is collecting the information in this electronic form for the purpose of assessing my entitlement to Commonwealth assistance under the Higher Education Support Act 2003 or the Research Training Program and allocation of a Commonwealth Higher Education Student Support Number (CHESSN) to me

• James Cook University will disclose this information to the Department of Education for those purposes

• The Department of Education will store the information securely in the Higher Education Information Management System

• The Department of Education may disclose the information to the Australian Taxation Office (ATO), and

• James Cook University and the Department of Education will not otherwise disclose the information without my consent unless required or authorised by law.

INFORMED CONSENT for INTERNATIONAL APPLICANTS

I understand that:

• James Cook University is collecting the information in this online form for assessment and administrative purposes

• James Cook University will not disclose personal information to any external bodies without my authorisation unless a valid legal request has been made.
