It is a JCU requirement that ALL professional placement students have a doctor complete the JCU Health Record and Immunisation form. The doctor cannot be related.
International Students must have their vaccination records translated into English and complete all forms in English.
Please provide a copy of the completed form to the Professional Experience Placement Unit. Retain a copy of the certificate in your professional portfolio
