JCU Health Record and Immunisation Form

All JCU students undertaking professional experience placements must have a medical practitioner complete the applicable sections of the JCU Health Record and Immunisation Form. The medical practitioner must not be a relative.

Please email a copy of the completed form and any available serology results to the Professional Experience Placement Unit.

Students can seek advice regarding their health and vaccinations from JCU Health General Practice, located on Level 1 of the Clinical Practice Building at JCU's Bebegu Yumba campus in Douglas, Townsville, or from your General Practitioner.

Vaccine-Preventable Disease Immunisation Requirements

In accordance with JCU requirements and applicable Queensland Health requirements, students enrolled in health courses must provide evidence of vaccination or immunity relevant to their placement role before commencing professional experience placement, or by the end of the first teaching period, whichever occurs first. The core vaccine-preventable disease requirements are outlined below.

Disease

Vaccination Evidence Required

Hepatitis B

Documented evidence of a completed hepatitis B vaccination course or evidence that you are not susceptible to hepatitis B.

Measles, mumps and rubella

Documented evidence of positive IgG antibodies for each disease on serology, or documented evidence of two MMR vaccinations administered at least one month apart.

Varicella

Documented evidence of positive varicella IgG antibodies on serology, or documented evidence of two varicella vaccinations administered at least one month apart.

Pertussis

Documented evidence of pertussis booster vaccination in the previous 10 years.

International students must have vaccination records that are not in English translated into English and must complete all required forms in English.

COVID-19 Vaccination

Although COVID-19 vaccination is not mandatory for all placements, it remains an important measure for reducing the risk of severe illness. Current guidance is available at Australian Government COVID-19 vaccination guidance (Australian Immunisation Handbook).

Some placement facilities may require COVID-19 vaccination as a condition of entry. Students will be advised of any facility-specific requirement when their placement is allocated.

Influenza Vaccination

Students undertaking a placement in a residential aged care or multipurpose health service, or visiting an aged care facility during placement, are strongly encouraged to have the current seasonal influenza vaccination before entry. A placement facility may also specify influenza vaccination as a condition of entry.

Although influenza vaccination is not mandatory for all placements, JCU strongly recommends that all students receive the current seasonal influenza vaccine.

If you cannot be vaccinated because of a medical condition, please obtain supporting documentation from your medical practitioner and email it to the address above.
