Troubleshooting - Student Application Forms

A technical issue has prevented some JCU student forms from operating correctly from 5.00 pm, Thursday, 23 May 2024. For some students, these errors have persisted since the last TechByte was issued in relation to this on 29th May 2024.

In particular, we are aware of errors with the Special Consideration application form.

To resolve this error and submit any of the forms listed below, please use this new form link in a new browser window.

If you are still experiencing submitting an application form, please contact the IT Helpdesk for further assistance.

The new form link will apply to the following forms:

  • Application for special consideration
  • Application for credit
  • Change of personal details
  • Request for USI
  • Appeal application – academic decisions
  • Appeal application – complaint and conduct decisions
  • Formal review application – academic or statutory decisions
  • Confirmation of enrolment
  • International letter request
  • Application for refund of overpayment or credit balance
  • OS-HELP loan application
  • Late subject enrolment
  • Outbound cross institutional study
  • Course maximum time extension
  • Request for GPA Indicative Letter of Advice
  • Request to change result type for TP1 2020
  • Request to withdraw from course JCU Brisbane

On opening the link, you will be asked to sign in using Student Single Sign-On:

Click on the log on button and sign into JCU Multi-factor Authentication.

MFA Logon

Once you have logged in, you will see this forms menu. Please select the one you need by choosing the appropriate Fill out button.  You will then see a pop up. Please select Create new form.
