I have completed the 'Credit Transfer Subject Equivalence Form' and/or the 'Informal/Non Formal Learning Profile' ready to be attached to the application.
I am ready to upload all required supporting documentation. I understand the 20 working day assessment period commences upon receipt of a complete application. Applications are deemed incomplete if documentation is missing, and may be withdrawn due to non-communication or lengthy response times.
If including my CV (resume) as supporting documentation, I have also completed a statutory declaration.
If applying for credit for the Bachelor of Veterinary Science, Bachelor of Dental Surgery or Bachelor of Medicine - Bachelor of Surgery, I already have an offer to commence study.
If applying for credit for the Master of Guidance and Counselling on the basis of professional experience, I have supplied a Statement of Service.

This personal informal may be disclosed to the educational institutions you have attended for verification of your previous studies in order to make an informed decision about the application. This personal information will be dealt with in accordance with the requirements of the Information Privacy Act 2009 (Qld), will not be disclosed to any other person or agency other than for the purpose stated above, if the disclosure is authorised or required by law or if you have given JCU permission for the information to be disclosed. Your rights to access and amend your personal information are set out in the Information Privacy Act 2009 (Qld) which also places obligations on JCU as to how we handle your personal information.
